SAN entries must be encoded in the request to the SAN extension OID = 2.5.29.17. Microsoft servers sometimes use a different format. In Microsoft Technet is available at http://technet.microsoft.com/de-de/library/ff625722(v=ws.10).aspx a detailed description of how the appropriate format can be generated.

A CSR (certificate signing request) or server certificate request is a certification request from the server on which the SSL certificate should be installed. The CSR is generally created by customers on the relevant machine and then manually entered in the ordering page. The syntax is described by the standard PKCS#10. The server applications make suitable creation tools available. The product-specific characteristics are to be taken into account when creating the request.
